Open to the Community COVID-19 Drive-Thru Vaccine Clinic

• Pfizer Vaccine
• Must be 16 years old or older
  • If you are 16 or 17 parental/guardian consent is required and your parent/guardian must attend the clinic with you
• You must be available on May 20th to receive the 2nd dose at the same location
• One sign-up is required for each individual
• Masks are required at all times.
• If you need to cancel, please let us know by April 28th so that we can fill your timeslot.
  • If you are sick or have a fever, please stay home and notify us ASAP.

FYI

• Health insurance is not required to receive the vaccine.
• Please only RSVP if you are sure you can make it to this date and the second dose date 28 days later.
