Starting the discussion early and being educated on your options can lead to a positive and successful outcome that ensures your loved one continues to receive the level of service and supports they deserve.

Transition Planning

Oftentimes a smooth transition plan will incorporate your loved one’s case manager and your in-home support services agency (like PASCO). Individual situations may require some legal assistance depending on your unique circumstance such as Guardianship, Special Needs Trust, and/or Financial/Estate Planning.

There are several options available in Colorado when it comes to waiver transitions. Based on feedback from our PASCO families, we have outlined a few key considerations below.

CHCBS to EBD

Children’s Home and Community-Based Services Waiver to Elderly, Blind, and Disabled Waiver

  • IHSS – if you are currently providing In-Home Support Services (IHSS) and want to continue this service when your loved one transitions into adulthood, you should look into the EBD waiver. IHSS is a service option included under the EBD waiver.
CHCBS to EBD Waiver

CES to SLS

Children’s Extensive Support Waiver to Supported Living Services Waiver

  • Family CNA – if you are currently providing CNA care to your loved one and want to continue this service you should look into the SLS waiver.
    • With the SLS waiver, you can expand the scope of care above Family CNA to include supported community connections (one on one support in the community), mentorship, homemaking, and respite.

CES to DD

Children’s Extensive Support Waiver to Developmental Disabilities Waiver

  • Family CNAs or Family Personal Care Providers – if you are currently providing either of these services to your loved one and want to continue you should look into Residential Services under the DD Waiver.
    • Benefits to the CES to DD waiver selection include:
      • Avoiding the DD waitlist, access to 24/7 services and supports, individualized contingency planning, access to day programs, and supported community connections (access to the community through paid caregiving).
    • Areas of concern – you will lose access to additional therapies such as music, massage, hippotherapy, and home and vehicle modification benefits.

Children’s Home and Community-Based Services Waiver to Elderly, Blind, and Disabled Waiver

  • Family CNA – if you are providing CNA care to your loved one and are looking to provide additional tasks, you may want to consider IHSS under the EBD waiver. The Nurse Practice Act is waived, allowing caregivers to provide additional paid services under IHSS.

Children’s Extensive Support Waiver to Supported Living Services Waiver

  • Family CNA – if you are currently providing CNA care to your loved one and want to continue this service you should look into the SLS waiver.

Children’s Extensive Support Waiver to Elderly, Blind, and Disabled Waiver

  • Access to IHSS (In-Home Support Services) – if you are providing CNA care to your loved one and are looking to provide additional paid tasks (medication administration, ostomy care, catheter care) you may want to consider IHSS under the EBD waiver. The Nurse Practice Act is waived allowing caregivers to provide additional services under IHSS.

Children’s Extensive Support Waiver to Developmental Disabilities Waiver

  • Benefits – Avoid DD waitlist, access to 24/7 services and supports, individualized contingency planning, PASCO advocacy for additional support funding, nursing delegation of complex skilled tasks including medication administration and g-tube training and support.

Children’s Home and Community-Based Services Waiver to Elderly, Blind, and Disabled Waiver to Developmental Disabilities Waiver

  • IHSS – if you are currently providing IHSS services and want to maintain continuity when your loved one transitions into adulthood, you could look into the EBD waiver while requesting to be placed on the DD waitlist through your case management agency.
    • Emergency DD enrollments can happen under qualifying circumstances allowing your loved one to access residential support services such as host homes for contingency planning.
    • PASCO can work with the individual’s case manager to plan for the future as part of our transitions support.

CES to SLS to DD

Children’s Extensive Support to Supported Living Services to Developmental Disabilities Waiver

  • Family CNA – if you are currently providing CNA care to your loved one and want to continue this service you should look into the SLS waiver while requesting to be placed on the DD waitlist through your case management agency.
    • Emergency DD enrollments can happen under qualifying circumstances allowing your loved one to access residential support services such as host homes for contingency planning.
  • PASCO works with the individual’s case manager to plan for the future.
