Quarterly Transition Conversation & Resource Exchange (QTC&RE)
PASCO will host and facilitate informative transition sessions targeted at families of 14-17 years old kids. The goal is to have everyone aware of nuances, timelines, services, and different (adult) Colorado Medicaid waivers. Furthermore, community partners will be on hand to help provide valuable resources. Bring your question and appetite, as lunch will be provided.
