      Job Description

      RN Case Manager

      Exemption Status: Non-Exempt

      Position Type: Variable hours based on caseload

      Reports to: Director of Nursing



      Job Summary

      Act as a comprehensive care coordinator for consumers assigned to his / her care, continually assessing the consumer’s needs, coordinating care amongst all providers and ensuring high quality care. Perform skilled nursing services, provide consumer education, complete and submit all required documentation, and supervise and direct assigned care providers.

      Key Duties and Responsibilities

      • Act as a comprehensive care coordinator for consumers assigned to his / her care, continually assessing the consumer’s needs, coordinating care amongst all providers and ensuring high quality care delivered within governing body rules and regulations
        • Establish and maintain relationships with assigned consumers
        • Perform comprehensive assessments of physical, emotional, psychosocial, and environmental needs of the consumer including outcome and assessment information
        • Develop and implement a realistic individualized Plan of Care (Physician’s Plan of Care, Home Health Aide Plan of Care) for each consumer, evaluate and revise this plan as necessary; interpret the appropriate information needed to identify each consumer’s requirements
        • Participate in development of consumer goals as well as make revisions to these goals based on the changes in consumer status.
        • Interpret and implement physicians orders accurately and promptly and demonstrate appropriate procedures as necessary to LPNs, CNAs or other care providers
        • Monitor visit frequency and duration by reviewing physicians’ orders and comparing to the Plan of Care and documentation
        • Identify significant clinical findings, make conclusions and intervene appropriately
        • Review results of laboratory tests and other diagnostic procedures and report to other care provider appropriately
        • On an ongoing basis, evaluate the effectiveness and outcomes of care and the appropriateness of the Plan of Care, and initiate necessary revisions with communication with all other disciplines
        • When unable to handle care (e.g. vacation or leave), make attempts to find a replacement and communicate status to Supervisor
      • Complete and submit all required documentation per governing body rules and regulations and within established Agency guidelines
      • Proactively schedule and perform consumer visits to meet deadlines
      • Prioritize nursing visits based on the consumer’s health status and anticipated needs.
      • Perform skilled nursing services under the orders of the consumer’s physician and in accordance with accepted standards of practice and governing body rules and regulations
      • Supervise and direct assigned LPN’s and aides per Federal, State and Local regulations and Agency Policy.
        • Ensure quality of care
        • Complete and process CNA or other care provider supervisory forms in appropriate timelines
        • Provide feedback in performance management process
        • Verify and document attendant skills and competency to perform IHSS and basic consumer safety procedures
        • Counsel attendant staff on difficult cases and potentially dangerous situations
        • Consult with the client, authorized representative or attendant in the event a medical issue arises
        • Investigate complaints and critical incidents within 10 working days
        • Assure that the attendant is following directives found in the IHSS Plan
      • Understand start of care, discharge, transfer, recertification and on-going client documentation; ensure time spent learning maximizes both efficiency and thoroughness of field documentation.
        • Understand all paperwork and care is completed per PASCO policy and applicable rules and regulations o Understand the Plan of Care is reflective of and appropriate to the assessment
        • Understand connection of Skilled Nurse, and CNA visit frequency matching physician’s orders
        • Under direction of QA Team, document issues and return paperwork to nurse for moderate to major corrections
        • Under direction of QA Team, follow up on problems or questions related to consumer documentation
      • Identify learning needs of the consumer and/or those providing care, educate consumers and family members (if applicable) and make appropriate referrals to community resources, enhancing the health and well-being of the consumer
      • Maintain strong two-way communication with team members and PASCO nursing administration.
      • Provide key information to Clinical Nursing Supervisor and physician (wounds, infections, hospitalizations, abnormal findings, etc.)
      • Attend nurse meetings and absorb information provided, or proactively acquire information presented through other means
      • Proactively communicate any other issues including (but not limited to) clinical questions or special circumstances, compliance questions or issues, etc.
      • Understand and comply with all appropriate HIPAA regulations, maintain strict consumer health care record privacy and confidentiality
      • Assume on-call responsibilities on a rotating or as needed basis

      • At all times, adhere to all rules and regulations set forth by State and Federal Agencies as well as PASCO’s administration.

      Qualifications

      Experience
      • 1 year experience practicing as an RN required, additional experience in acute care, medical / surgical nursing strongly preferred
      • Experience and skillset matching assignments
      • Home health nursing experience preferred
      Education and/or Licenses
      • Current license to practice as a Licensed Practical Nurse in the state of Colorado
      • Graduation from a state approved School of Practical Nursing
      • Current CPR / first aid certification
      Knowledge, Skills and Abilities
      • Clinical Judgment: Ability to gather data about a consumer and make judgments regarding when and what to report to the primary RN or other care providers.
      • Consumer Service: Ability to establish and maintain relationships with consumers, and provide professional appropriate, compassionate and empathetic care to consumers
      • Maintaining Commitments: Ability to consistently fulfill commitments, show up when promised, and maintain strong communication

      Skilled Visit Nurse, LPN

      Exemption Status: Non-Exempt

      Reports to: Clinical Manager and RN Case Manager



      Job Summary

      Provide selected home health nursing services within the LPN scope of practice, with guidance and supervision from a Clinical Manager and RN Case Manager.

      Key Duties and Responsibilities

      • Perform selected skilled nursing services within the LPN scope of practice, under the direction of the RN Case Manager as well as orders of the consumer’s physician and in accordance with governing body rules and regulations.
        • Implement physicians orders promptly and accurately, follow the plan of care
        • Assist the registered nurse in performing specialized procedures, prepare equipment and materials for treatments observing aseptic technique as required
        • Establish and maintain relationships with assigned consumers
      • Ensure high quality and high continuity of care by communicating and coordinating with RN Case Manager and physician, both orally and through documentation
        • Observe, record, and report to appropriate supervisory personnel key information (wounds, infections, hospitalizations, etc.), as well as the general physical and mental condition of the consumer, and signs and symptoms which may be indicative of condition changes
        • Complete and submit all required documentation per governing body rules and regulations and within 48 hours
        • Thoroughly understand the consumer’s individualized Plan of Care (Physician’s Plan of Care, Home Health Aide Plan of Care)
      • Educate consumers and family members (if applicable) in self-care techniques.
      • Maintain strong two-way communication with team members and PASCO nursing administration.
      • Attend nurse meetings and absorb information provided, or proactively acquire information presented through other means
      • Proactively communicate any other issues including (but not limited to) clinical questions or special circumstances, compliance questions or issues, etc.
      • Understand and comply with all appropriate HIPAA regulations, maintain strict consumer health care record privacy and confidentiality
      • At all times, adhere to all rules and regulations set forth by State and Federal Agencies as well as PASCO’s administration.
      • Other duties as assigned by PASCO’s administration.

      Private Duty Nurse, LPN

      Job Summary

      Provide selected home health nursing services within the LPN scope of practice for a single client on an hourly basis, with guidance and supervision from a Registered Nurse.

      Key Duties and Responsibilities

      • Perform selected skilled nursing services within the LPN scope of practice on an hourly basis, under the direction of the RN Case Manager as well as orders of the client’s physician and in accordance with governing body rules and regulations
        • Implement physicians orders promptly and accurately, follow the plan of care
        • Assist the Registered Nurse in performing specialized procedures, prepare equipment and materials for treatments observing aseptic technique as required
        • Establish and maintain relationships with assigned clients and families
      • Ensure high quality and high continuity of care by communicating and coordinating with RN Case Manager and physician, both orally and through documentation
        • Observe, record, and report to appropriate supervisory personnel key information (wounds, infections, hospitalizations, etc.), as well as the general physical and mental condition of the client, and signs and symptoms which may be indicative of condition changes
        • Complete and submit all required documentation per governing body rules and regulations and within established Agency guidelines
        • Thoroughly understand the client’s individualized Plan of Care (Physician’s Plan of Care, Home Health Aide Plan of Care)

      Support independent living skills and promote choice while assisting persons with disabilities to live productive and meaningful lives. Provide services to individuals with disabilities per the Plan of Care and within the scope of practice based on certification and compliance with the specific waiver program. Based on assigned client you may provide services in any or all of the waiver services programs below.

      Key Duties and Responsibilities

      IDD Direct Service Provider (DSP)
      • Develop opportunities to promote independence in the home and community
      • Assist individuals with I/DD to live life in their homes and communities.
      • Work with individual receiving services to achieve the goals outlined in their Service Plan and Individual Service and Support Plan.
      • Assist individuals with I/DD to live life in their homes and communities.
      • Establish and maintain relationships with individuals receiving services

        • The general condition and safety of the consumer’s physical environment, and any changes that could adversely affect the consumer’s health or safety
        • Any other incidents or concerns, including but not limited to: falls or injuries or evidence of an injury, missed medications, no food on the premises, etc.
      • Maintain timely records of home visits per PASCO policy and governing body rules and regulations
      • Attend and participate in appropriate consumer care conferences, staff meetings and agency committee meetings
      • Empty ostomy and catheter bags
      • Remind consumers to take own medications

      Health Maintenance Attendant
      • Perform all functions that IHSS Field RN has assigned in the Plan of Care, for which the HMA has demonstrated competency and been validated by the IHSS Field RN
      • Establish and maintain relationships with assigned consumers
      • Observe, record, and report to appropriate supervisory personnel:
        • The general physical and mental condition of the consumer, and signs and symptoms which may be indicative of condition changes
        • The general condition and safety of the consumer’s physical environment, and any changes that could adversely affect the consumer’s health or safety
        • Any other incidents or concerns, including but not limited to: falls or injuries or evidence of an injury, missed medications, no food on the premises, etc.
      • Maintain timely records of home visits per PASCO policy and governing body rules and regulations
      • Attend and participate in appropriate consumer care conferences, staff meetings and agency committee meetings

        • Mouth care
        • Skin and nail care
      • Prepare meals
      • Assist with feeding
      • Remind consumers to take own medications
      • Remind consumer to perform exercise program
      • Perform household tasks per the consumer’s direction including consumer’s laundry, light housekeeping, shopping, errands, etc.
      • Observe, record, and report to appropriate supervisory personnel:
        • The general physical and mental condition of the consumer, and signs and symptoms which may be indicative of condition changes
        • The general condition and safety of the consumer’s physical environment, and any changes that could adversely affect the consumer’s health or safety

      Backup CNA Caregiver

      Job Summary

      On a shift scheduled basis, provide personal assistance services to individuals with disabilities per the Plan of Care, within the CNA scope of practice and under the direction of the Skilled Services Coordinators and Senior Manager of Clinical Operations.

      Key Duties and Responsibilities

      • Maintain availability via cell phone while on duty, and respond to calls in a timely manner.
      • Under the direction of the Skilled Services Coordinators and Sr. Manager of Clinical Operations, assist in any way needed during assigned shift; this includes but is not limited to performing scheduled or extra consumer visits
      • Perform a wide variety of skilled and unskilled tasks as outlined in the Consumer’s Plan of Care, including but not limited to CNA, PCW, IHSS and IDD roles
      • Maintain timely records of home visits per PASCO policy and governing body rules and regulations.
      • Maintain strong two-way communication with Skilled Coordinators, On-Call Nurse and PASCO administration
      • Proactively communicate any issues including client concerns, clinical questions, special circumstances, compliance questions or issues, etc.
      • Understand and comply with all appropriate HIPAA regulations, maintain strict consumer healthcare record privacy and confidentiality
      • At all times, adhere to all rules and regulations set forth by State and Federal Agencies as well as PASCO’s administration
      • Attend assigned meetings and/or proactively acquire information presented through other means
      • Other duties as assigned by PASCO’s administration.

      Qualifications

      Experience
      • CNA and/or Home Health experience preferred
      Education and/or Licenses
      • Current license to practice as a Certified Nurse Assistant in the state of Colorado
      • Graduation from a state approved Certified Nursing Assistant training program
      • High school diploma or equivalent preferred
      • CPR training (PASCO will pay for this)
      • Must maintain 12 CEUs per year
