Step 1 of 8
Act as a comprehensive care coordinator for consumers assigned to his / her care, continually assessing the consumer’s needs, coordinating care amongst all providers and ensuring high quality care. Perform skilled nursing services, provide consumer education, complete and submit all required documentation, and supervise and direct assigned care providers.
Ethical: We always try to do the right thing: We follow all applicable laws and regulations. We try to make ethical decisions and act with integrity. When serving consumers, we listen to and try to serve the consumers best interest. When we find a problem, we openly acknowledge and fix the problem.
Advocacy: We are advocates: We support consumer independence. We always try to see both sides of any issue and advocate for positions that support consumers, employees, and each other.
Learning: We are a learning organization: We strive to constantly improve how we serve our consumers, and operate the business. We have an open mind about new ways of doing what we do and are adaptable to the changing business and service environment. We learn from our mistakes. We reflect on and assess the results of any idea we implement, continuously improving based on the assessment. We look for development opportunities to constantly improve our professional skills.
Accountable: We are accountable: When we say we are going to do something we do it. But we are not just accountable for each assigned task or project. We are accountable for ensuring our business operates in a manner that is compliant, consumer/employee focused, and successful. When we see an issue we do not say “that is not part of my job.” We either own the issue to resolution or make sure the correct individual is alerted to the issue and takes ownership for resolution. We check back in to make sure the issue was resolved.
Team Work: We approach challenges as a team: When we identify a problem or opportunity, we approach it as a team. When we encounter an issue, the first question we ask is “Who should be involved in this conversation.” We are respectful of each individual’s ideas and suggestions. We are open to feedback from the team, and are comfortable providing feedback to the team.
