THRIVE Medical Transition to Adult Care
“ECHO for Families” community learning session for parents and primary caregivers of children and youth with special health care needs (CYSHCN) or intellectual/developmental disabilities (I/DD). This interactive Zoom session will discuss the medical transition for adolescents as they enter the age of autonomy and provide information to help families make informed decisions.
