We are excited to invite you to our free PASCO Transition Fair. When your loved one approaches adulthood, navigating Medicaid and the transition process can be overwhelming. Our PASCO teams are here to make Medicaid Made Easy!

Transitioning within PASCO means you will maintain your tenure and the benefits you currently receive with PASCO, all while providing continuity of care between our service lines.

We will be discussing the following useful topics:
• Guardianship presentation with Chris Brock, Managing Attorney of Probate Power at the Colorado Cross-Disability Coalition
• Long-Term Home Health (LTHH) Medicaid
• Adult waivers and their transition options
