Transitioning within PASCO means you will maintain your tenure and the benefits you currently receive with PASCO, all while providing continuity of care between our service lines.
Participants will receive a 1 on 1 informational session with our Client Services team. They will be discussing the following topics:
• Long-Term Home Health (LTHH) Medicaid
• Adult waivers and their transition options
• Useful PASCO resources
